Terms of Use

Form is a workout and activity journal with an AI-assisted training companion. By creating an account or using the app, you agree to these terms.

The service

You describe training and Form structures it into workouts, exercises, sets, plans, summaries, and coaching suggestions. AI output can be incomplete or wrong. Review your journal before relying on it.

Health and safety

Form is for general fitness journaling and education. It is not medical care, diagnosis, physical therapy, emergency guidance, or a substitute for a qualified clinician or trainer who knows you. Stop an activity that causes pain, seek appropriate professional advice, and do not use Form to make medical decisions. You remain responsible for exercise selection, technique, loading, and safety.

Your account

You sign in with Apple and are responsible for activity on your account. You must be at least 13, or the minimum age required in your country, to use Form.

Subscriptions

Paid features require a subscription purchased through Apple. Payment is charged to your Apple account and subscriptions renew automatically unless cancelled at least 24 hours before the end of the current period. Manage or cancel in device Settings → Apple Account → Subscriptions. Refunds are handled by Apple under its terms.

Deleting your Form account deletes Form data but does not cancel an Apple subscription. Cancel the subscription separately through Apple if you do not want it to renew.

Your content and AI processing

The workout details, preferences, and messages you enter remain yours. Before entering the app, you choose whether to permit Form to store and process this content and to send relevant training and health context through OpenRouter and selected AI model providers. This processing is necessary for the AI-assisted service; if you do not agree, you cannot use Form. The privacy policy explains the data and providers in more detail, including how hashed cache identifiers and generated artwork may be reused in an account-independent exercise-art library. Do not put personal information in exercise names. You can export or delete your account-linked data in Profile.

Fair use

Do not attempt to break, overload, scrape, reverse engineer, or misuse the service; upload illegal or harmful content; interfere with another person's account; or use the AI features for unrelated automated workloads. We may suspend accounts that abuse the service.

Availability and disclaimers

Form is provided “as is” and may change or experience interruptions. To the fullest extent allowed by law, we disclaim warranties of any kind and are not liable for indirect or consequential damages arising from use of the app. Where liability cannot be excluded, our total liability is limited to the amount you paid for Form in the twelve months before the claim.

Ending things

You may stop using Form and delete your account at any time in the app. We may suspend or end access for accounts that violate these terms. Sections that by their nature should survive termination remain in effect.
